Who Qualifies

Am I eligible
for Medicaid?

Medicaid eligibility is based primarily on income and household size. The following groups are most commonly eligible under state and federal guidelines.

Low-Income Adults

Adults aged 19–64 with household income at or below 138% of the Federal Poverty Level (FPL) may qualify in states that have expanded Medicaid under the Affordable Care Act.

Children & Families

Children in families with low to moderate incomes may qualify for Medicaid or the Children's Health Insurance Program (CHIP). Coverage includes routine check-ups, immunizations, dental, and vision.

Pregnant Women

Pregnant women with income up to 138–200% FPL (varies by state) qualify for Medicaid coverage that includes prenatal care, labor and delivery, and postpartum care for up to 12 months.

People with Disabilities

Individuals who receive Supplemental Security Income (SSI) automatically qualify for Medicaid in most states. Others with qualifying disabilities may also be eligible based on income and functional need.

Seniors (Dual Eligible)

Individuals who qualify for both Medicare and Medicaid are called "dual eligibles." Lifebridge Health offers Dual Special Needs Plans (D-SNP) that coordinate both programs to minimize out-of-pocket costs.

Foster Care Youth

Young adults who aged out of foster care are eligible for Medicaid coverage up to age 26 regardless of income in most states, ensuring continuity of care during the transition to adulthood.

Prescription Drugs

Broad formulary covering thousands of generic and brand-name medications. Most prescriptions are available at $0 or very low copays at preferred pharmacies.

Dental & Vision

Routine dental exams, cleanings, and fillings plus annual eye exams and eyeglass allowances are included in most Lifebridge Medicaid plans.

Mental Health & Substance Use

Behavioral health services including therapy, counseling, psychiatric care, and substance use disorder treatment — covered at parity with medical benefits.

Long-Term Care

Nursing facility care, home and community-based services (HCBS), personal care assistance, and adult day health services for members who need ongoing support.

Enrollment

How to apply
for Medicaid

Applying for Medicaid is free and can be done online, by phone, or in person. Lifebridge Health enrollment specialists are available to guide you through every step.

Step 1

Check your eligibility using our online screening tool or call our enrollment hotline at 1-800-524-2273.

Step 2

Complete your application with proof of income, residency, and identity. Our specialists can help you gather the required documents.

Step 3

Your state Medicaid agency reviews your application. Most decisions are made within 45 days (90 days for disability-based applications).

Step 4

Once approved, choose Lifebridge Health as your managed care plan and receive your member ID card within 7–10 business days.

Medicaid FAQs

Medicare is a federal health insurance program primarily for people 65 and older, regardless of income. Medicaid is a joint federal and state program that provides health coverage to low-income individuals and families of all ages. Some people qualify for both programs simultaneously.
Dental and vision coverage for adults varies by state. Federal law requires dental and vision coverage for children through CHIP and Medicaid. Lifebridge Health Medicaid managed care plans include dental and vision benefits for adults in the states where we operate.
You can continue seeing your current doctor if they accept Medicaid and are in the Lifebridge Health network. Our network includes over 1,200 providers. If your doctor is not in our network, we can help you find a comparable provider nearby.
You must report income changes to your state Medicaid agency. If your income increases above the eligibility threshold, you may lose Medicaid coverage but could qualify for subsidized marketplace coverage. Lifebridge Health can help you transition to a new plan seamlessly.
Unlike Medicare, Medicaid has no set enrollment period. You can apply at any time of year. If you qualify, coverage can begin as early as the first day of the month you apply, and in some cases may be retroactive up to 3 months.
